Hello, please make the ICD-10 code field (box) a mandatory field. We have MD offices who are only typing in the diagnosis description and not providing the code. We (OR scheduler's) have to google and chose. This allows for code specificity to be inaccurate between what the office will wind up submitting in the future to insurance, and what we have documented. The MD diagnosis, and the corresponding code, should also be assigned by the MD (his assistant/office personnel).

Examples of diagnosis I have been given without codes:

1) Retained Foreign body of left shin, with surrounding inflammation.

2) Anastomotic stricture, status-post sigmoid colectomy, approximately 12 CM from the anal verge.

3) Serrated dysplastic adenoma at the level of the dentate line.
